The Opportunity
Description
We're looking for a Home Health Registered Nurse - Case Manager, working in Healthcare Systems and Services industry in Littleton, New Hampshire, United States.- Provide care to medical/surgical patients utilizing all components of the nursing process (assesses, develops, plans, implements, and evaluates) in the plan of care.
- Perform skilled observation and assessment.
- Provide wound care.
- Administer injections and IV therapy.
- Manage nursing cases effectively through case management procedures.
- Provide Home Health & Hospice services to patients of all ages.

Experience Required for Your Success
- Minimum of 2 years current Home Health experience in Case Management.
- NH RN or Compact RN License required.
- BLS required.
- Must be willing to work on Thanksgiving holiday.
- Must be willing to work every Friday.
- Must be willing to work every other weekend (If Needed/Scheduled).
