The Opportunity
Description
We're looking for a Registered Nurse Case Manager, working in Healthcare Systems and Services industry in Lanham, Maryland, 20706, United States.- Assess and reassess nursing needs, including the physical, psychosocial, and environmental needs of clients and their families.
- Develop an appropriate and individual care plan, collaborate with the client’s primary physician as needed.
- Teach/counsel clients regarding self-care and evaluating client response to care provided.

Experience Required for Your Success
- 2 years of nursing experience
- 1 year of home health care experience
- BLS and ACLS certification
- Active state license as RN
- Must be vaccinated Valid driver’s license
- Available personal vehicle, and proof of current auto insurance
