The Opportunity
Description
We're looking for a Nurse Practitioner, working in Healthcare Systems and Services industry in Philadelphia, Pennsylvania, United States.- Perform health risk assessments which are essentially history and physicals done within the patient’s home or on occasion in nursing homes or other health facilities.
- Ensure all appropriate consent forms are signed and dated.
- Educate patients and/or the patient's family on chronic medical conditions, preventative care, and medication adherence compliance.
- Complete referrals to case management, nursing, pharmacy, etc. as needed.
- Participate in clinical meetings and compliance training which are primarily web based; however, providers will occasionally be required to attend on-site training for which the provider will be reimbursed.
- Obtain samples for diagnostic screening tests such as finger stick for HgbA1C, LDL, and Lead levels; urine analysis may also be required based on client need.
- Review patient records for past and present health issues.
- Collaborate with the supervising physician and other members of the health care team.
- Interpret laboratory data and diagnostic testing when available to assist in diagnosis of medical conditions.
- Perform ADL and Fall Risk evaluation, Depression and Nutritional Health Screening, BMI evaluation, and medication reconciliation.
- Perform other duties as assigned by supervising physician.

Experience Required for Your Success
- 1-2 years of Nurse Practitioner experience
- Active CRNP PA License
- NPPES NPI Registration
- Medicaid Enrolled in PA
- Fully Vaccinated against Covid-19
