The Opportunity

Description

We're looking for a Nurse Practitioner, working in Healthcare Systems and Services industry in Georgia, United States.

  • Perform health risk assessments which are essentially history and physicals done within the patient’s home or on occasion in nursing home or other health facility; on minimal occasion some of these assessments are done through a telehealth visit using simultaneous video and audio communication.
  • Document acute, chronic, and long-term healthcare issues through patient encounter in EMR system; Documentation must be accurate, high quality, and completed in a timely manner
  • Ensure all appropriate consent forms are signed and dated
  • Have frequent interaction with patients, coders, schedulers, and supervising physician
  • Educate patients and/or patient’s family on chronic medical conditions, preventative care, and medication adherence compliance
  • Complete referrals to case management, nursing, pharmacy, etc. as needed
  • Participate in clinical meetings and compliance trainings which are primarily web based; however, provider will occasionally be required to attend on-site training
  • Occasionally obtain samples for diagnostic screening tests such as finger stick for HgbA1C, LDL, and Lead levels; urine analysis may also be required based on client need.
  • Review patient records for past and present health issues
  • Collaborate with the supervising physician and other members of health care team
  • Interpret laboratory data and diagnostic testing when available to assist in diagnosis of medical conditions
  • Do ADL and Fall Risk evaluation, Depression and Nutritional Health Screening, BMI evaluation, and medication reconciliation
  • Perform other duties as assigned by supervising physician such as performing examinations using equipment for diabetic retinal exams, bone density, etc.

Experience Required for Your Success

  • Must have background in primary care
  • Candidates must be board certified by the ANCC or AANP
  • Medicaid Enrolled in Georgia and Medicare enrolled. If not, must be willing to enroll
  • Must have active/unrestricted license in Georgia
  • Must have current BLS certification thru AHA
  • Must be vaccinated for Covid-19
  • Candidate must be willing to travel up to a 100 miles radius from their home zip code. (mileage will be reimburse)
  • Must use of own car with adequate automobile insurance
